Identified by Dr. Carlos Sandoval who provided a catalogue image of the Judet (pictured) and by Kevin Brown ("Devce Nation" poscast) who made the following comments:  "Judet cementless stem, early 1970s vintage.  Bring the Shukla and the Midas for this beauty as I believe the design was abandoned because they couldn't get it out.  Fun fact:  It was Judet who was credited with the first cemented total hip and that implant construct is THIS!!.
